Acupuncture Comprehensive Health History Form:

 
The health history form will help Lisa understand your main health issues and goals, and your overall health.  This will help her to individually tailor your intake and develop the treatment plan for your first treatment.

All new patients please complete the Health History and upload it to the secure server or bring it to your first appointment.

Life coaching intake and agreements will be emailed after scheduling.
 
Please review the Office Policies, Acupuncture Consent Form, and Privacy Practices before your first visit.

You will sign a consent form and notice of privacy practices at your first appointment.
